Few schools offer it, slots are scarce, and offerings swing year to year, so there is no copy-paste national list. The real gate is usually a natural-science/life-science written exam, plus making a verifiable case in your 志望理由 and 面接 for why you're switching, what you studied before, and how it converts into medical ability.
